CASE 16

 

PATIENT:  36-year-old white woman

 

CHIEF COMPLAINT:  This patient was admitted to the hospital at 21 weeks of gestation with an episode of deep vein thrombosis.

 

MEDICAL HISTORY:  Patient had a diagnosis of Systemic Lupus Erythematosus (SLE) and Lupus Anticoagulant about five years prior to admission.  She was noted approximately one week previously to have some manifestations of oligohydramnios and intrauterine growth retardation (IUGR) by ultrasound imaging.

 

FAMILY HISTORY:  Significant only for maternal hypertension.

 

DRUG HISTORY:  At the time of admission, the patient was on low-dose Aspirin, Prednisone 5 mg daily.

 

PHYSICAL EXAMINATION:  Not remarkable.

HOSPITAL COURSE:

Patient was started on heparin therapy.  Coagulation laboratory was consulted on laboratory monitoring of heparin treatment.
